What Is a Host Home?
A Host Home is a private residence where a qualified caregiver—known as a Host Home Provider—opens their home to care for an individual with special needs. This model provides a meaningful alternative to group homes, allowing for greater independence, personal attention, and community involvement in a family setting.

In each Host Home, individuals supported by Holistic Home Health LLC receive:
- 24/7 personalized care based on their unique needs
- Assistance with daily living activities such as bathing, meals, hygiene, and medication reminders
